Serious untoward incidents (S.U.I.s) are any extraordinary or unexpected incidents that have caused, or have the potential to cause, serious harm, or have, or are likely to have, attracted adverse public and media interest.  All services have serious untoward incidents upon occasion, no matter how effective the service may be.
